ECA Waiver Form

Policy of Guest Pass and Use of Facility 

• Everyone must register at the desk. 
• You must be a member or registered guest to be on the workout floor. (even to watch)
• Workout attire, including shirt and shoes, must be worn at all times.
• Smoking and the presence of alcohol or other intoxicants are not permitted on the premises. 
• Abuse of equipment and/or staff or obnoxious behavior will not be tolerated. 
• There is a $25 TRAIL fee for NON-MEMBERS. 
• Anyone using the facilities may be subject to having photos and/or video taken of them. 
• Escondido Combat Academy is not responsible for personal property left on the premises. 
• Anyone below the age of 18 years is required to have a parents or legal guardian present to use the facility. 

Risk Assumption and Waiver of Liability 

I acknowledge that participation in the activities offered by Escondido Combat Academy ("Club") may have an inherent risk of injury, financial loss or death. I hereby knowingly and voluntarily assume the sole risk, liability, and responsibility for any injury, financial loss or death resulting from my presence in the Club, or use of the Club's facility, instruction, or equipment, even when due to active or passive negligence by the Club and its owners, officers, or agents. I and on behalf of my executors, administrators, heirs, assigns, and successors, do hereby forever release and discharge the Club, its owners, principles, officers, employees, agents, trainers, servants, contractors, successors, insurers, and all other persons forms or entities, from any and all claims, demands, injuries, actions or causes of action, costs, loss of service, expenses and compensation whatsoever, whether known or unknown, foreseen or unforeseen, anticipated or unanticipated, that may exist.
